New Clients

If you're considering becoming a client the first thing we'll need is a completed screening form.
We use this form to start determining if our practice is a good match for your needs

Please note that we currently have a waitlist which varies in length depending on the requested provider.

Once the complete screening is received, we will reach out to you to ask any clarifying questions and to talk about the next steps.

Cancellation and Rescheduling

Our primary point of contact for all scheduling is our administrative staff who can be reached either by email at admin@alliancementalhealth.org or by phone at 603-343-2166 option 1.

If you need to schedule or reschedule an existing appointment please reach out to our administrative staff by one of the above contact methods. Please do not reach out directly to your provider.

We do ask for a notice period of 1 business day for any cancellation or appointment reschedule.

 

Missed appointments and Late change / cancellations for any reason incur the following fees:

  • Change or cancel with more than 24 hours notice - no charge

  • Change or cancel with fewer than 24 hours notice or appointments missed due to late arrival - $65

  • No show or appointment missed with no notice or contact - Full Fee for the scheduled service

Please remember that changing your appointment to an online format rather than missing it entirely could be an option for you and can be done on the day of the appointment without incurring any fees.
